Sounds like a reverse relay problem.  It's a common failure point - people 
reverse without waiting for the motor to stop fully, and the inductive 
surge 
burns the contacts.

If the relay still has 2 good contacts, you may be able to swap it with 
the 
FW relay and keep going for a while.  If not, Bill Gunn sells new reverse 
relays.  They're not terribly expensive - maybe $15.  

You definitely need a Homeowner's Service Manual, though.
