I'm almost certain that your 'forward/reverse' problem is in the reversing 
relay.  I used to have a lot of trouble with that until I quit using 
reverse for a brake.  Anyway the relay 'points' (contacts) get burned 
sometimes, then stick.  If it's not too bad you can file the points. 
Otherwise replace the relay.
