Sounds like the reverse relay contacts are burnt.  The symptoms suggest 
little or no shunt field power in reverse.  This is a very common problem. 
 

If your FW relay is identical to the reverse relay (mine was), I think you 
can temporarily exchange them to see if the symptoms disappear.  

A warning - the motor armature should never be powered up without an 
operating field.  You will overheat and damage the motor if you keep 
trying 
to run it that way.  I hope you haven't already done so.  (IMO, GE should 
have included some kind of safety circuit to inhibit start contactor 
pull-in 
unless it detected normal field current.)
