Contact a lift truck dealer.  I got a box from them and they had them in
stock. 
I think this is the maker Hyster, Unisource(TM)
 The part number is 3015692. Description: Bulb 36v B9.  The only
markings on the bulb are "Taiwan" and the code "SP5".

Also Radio Shack carries the dash bulb in 36v.
Part number: RSU-11385994.
