Using a voltmeter, check at the motor field terminals to make sure the 
field 
polarity is actually reversing when the relay pulls in.  I suspect that it 
isn't.  If not, it could be that the relay is clicking but the contacts 
are 
welded.  Another possibility is that the relay is somehow bypassed.

At one point, I had a replacement reverse relay I bought from Bill Gunn.  
It 
clicked, but didn't actually pull in.
